REQUIREMENTS FOR PROVISIONAL REGISTRATION

The Applicant is required to submit the following documents in original with its photocopies :

1. High School / H.S.S. mark sheet / Certificate in support of Date of birth
2. Final M.B.B.S. mark sheet of Part I & II.
3. Internship under doing certificate in original from the Dean of concerned Medical College
4. 2 Recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
5. For registration fees & penalty amount, please see the prescribed fees chart.
6. The Registered Medical Practitioner of Chhattisgarh Medical Council is required to attach a letter of N.O.C. of Chhattisgarh Medical Council.
7. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.

REQUIREMENTS FOR OBTAINING DUPLICATE REGISTRATION

The Applicant is required to submit the following documents in original with its 2 photocopies:

1. Photocopy of Registration Certificate
2. High School / Higher Secondary mark sheet in support of Date of birth,
3. Final M.B.B.S. mark sheet of Part I& II,
4. Internship Completion Certificate from the Dean / Principal of the Medical College.
5. M.B.B.S. Degree / Provisional Degree.
6. F.I.R. (for lost / Theft / missing / accident)
7. An Affidavit in the enclosed Proforma –P
8. 2 recent passport size Color photograph (not more than of 5cm x 4 –1/2 cm)
9. The prescribed fee (see fee chart).
